Replacement DEC District 8 Card (Pink)
A replacement DEC-District 8 Card cannot be issued to a student unless the following requirements have been completed.
- Student has passed Classroom Driver Education at Chantilly HS > Verified by Student View in SIS
- Student has attended the Partners for Safe Teen Driving > Verified through SIS by administration or through FCPS Adult and Community Education
- Student has passed summative final exam
- Online PE or Classroom Driver Education taken outside of Chantilly HS is not permitted to receive a reissue from Chantilly HS.
To apply for a replacement card:
Current Chantilly High School Students (Sophomores, Juniors and Seniors)
- Complete this DEC-8 Reissue form to apply for a Reissue card
- Bring $10 cash or Check (made out to: Chantilly High School) to the main office in an envelope for Drivers Education mailbox
- An email will be sent to you when it is ready for pick up
- NOTE – During the summer months, processing time for the replacement card may be up to 30 days. Please plan accordingly.
Alumni of Chantilly High School (younger than 20 years of age)
- You must have completed the Driver’s Ed course with Chantilly High School
- If you took Driver Education at Chantilly and are no longer a student at Chantilly, please visit Chantilly HS in person to complete a replacement form. Replacements are not issued same day.
- Online PE or Classroom Driver Education taken outside of Chantilly HS is not permitted to receive a reissue from Chantilly HS. Please contact your online teacher for a reissue.
- An email will be sent to you when it is ready for pick up
- When picking up the new card – student must bring $10 cash or Check (made out to: Chantilly High School) in an envelope for Driver's Education to the main office at Chantilly High School
- NOTE – Processing the request requires research through archived records and will take a minimum of 10 school days to be completed. If this is during the summer months, the wait time could be as long as 30 days. Please plan accordingly.
