Below you will find the forms you will need to start the new school year!
REQUIRED FORMS FOR ALL STUDENTS
Emergency Care Form
- All students must have accurate emergency care information. Parents update this information via their SIS ParentVue account.
- Log into your SIS ParentVUE account
- In order to log into Parent View it is necessary to have an active SIS Parent Account - if you need a SIS Parent Account, please contact Chantilly HS. If you are new to FCPS you will receive an activation letter in the mail.
- Parents who do not have a SIS ParentVUE account continue to have the option to communicate updates to their student’s school via the normal paper process.
- Parents can print and complete a blank Emergency Care Information Form needed for updates and submit it to their student’s school.
Health Information
- A health information form must be on file for all students. If you completed/updated the Online Verification/Update (OVU) your health information will automatically be updated. If you would like to complete a hard copy, send it via email to [email protected] or mail/drop-off at school.

Digital Resource Consent
Fairfax County Public Schools (FCPS) uses a variety of resources to support student learning. FCPS conducts the same technical evaluations for all products that use student information. In cases where FCPS contracts with a vendor to host student information, FCPS requires that the vendor adheres to the security and privacy requirements specified in a confidentiality agreement included in their contracts. Some of the digital resources your child may use this year require parental consent. Your child may use digital tools that do not collect, use or share any personal information. Please complete the Parent Digital Consent Application.

FCPS Privacy and Opt-Out Forms
- Details parents' rights regarding FCPS policies. Complete these forms only if you wish for your child to be opted-out. Please submit to Sue McCallister at [email protected]

- If you wish to opt your student out of an FCPS student device, please login to ParentVue, select the Student Info button and edit the information in the Accept FCPS Device section.

Fairfax County Public Schools wants all parents to understand its policies regarding surveys, records, curriculum, privacy, and related rights.
In its Annual Notice of Survey, Records, Curriculum, Privacy, and Related Rights and Opt-Out Forms (Annual Notice), FCPS provides explanations of the rights listed above as well as forms that may be used to exercise opt-out choices. Two versions of the Annual Notice are available — a grades K-8 version and a grades 9-12 version. Parents with children at different grade levels should read each applicable version of the Annual Notice because the notices and opt-out rights vary depending on the student’s grade level.

Student's Rights and Responsibilities

Log into your SIS ParentVUE account to review your students information and opt-out of using an FCPS laptop if your student wishes to use a personal device instead.

Bring Your Own Device (BYOD)
- Please, review the BYOD Guidelines if you are considering to opt-out of using an FCPS issued device.
- Parents can use this online payment system to check balances and add money to school cafeteria accounts, to make payments to the school for various school related items and to receive low account balance notifications.
